Hello, I purchased a ttc girl plan and thought it included the personalized portion. I've been trying to find the questionnaire but can't seem to find it. Can someone please help me? Thanks so much

Sorry to be a pain but I cannot find this questionaire anywhere. I have logged out and back in and also tried to click on links provided for others who couldn't find it and nothing comes up other than links for Drs etc. Any help would be greatly appreciated
