How do I purchase a customized sway plan?

Hi! I am looking to purchase a customized girl sway plan, but I can't figure out how to do that. I only see the option for the standard plan/diet. Any help? Thanks!

At the top of this page, where it says "membership and plans" or here is the link just to make it easier: https://www.genderdreaming.com/forum/payments.php you get the options to do a custom plan. Thank you!

Thank you, I purchased a plan! Is the questionnaire sent to me or do I find it on a thread somewhere? Sorry for so many questions, I've searched around and just haven't been able to find the answer.
