Is your coaching service still active?? It's much easier for you to post questions in the same place where your plan is so I can refer back to the plan and your questionnaire and see what you're taking.

If you prefer to continue here, can you tell me what you're taking?

When I go to that page, there are three questionaries for doctors, then two for attempts... which one is for me? I thought the first three are for IVF/PDG?
