Did I lose my pre questionnaire form?

I was filling out the pre-form questionnaire and my computer froze. I had previewed it a few times, but it wasn't complete. (Insert SAD face). I had to close the page. Did I lose it or am I lucky enough that it was stored somewhere in my profile?

You can delete this whole thing. I did lose it. I redid it and now realized that my completed forms are in my profile. Thanks!
