Still can't find custom plan questionnaire?

Sorry to post this bc I see lots of forum posts about it, but I purchased the custom plan and can't for the life of me access the questionnaire or find it! Any advice?

I can't find the questionnaire, I've clicked the link and all I see are results for sways and consult forms, this is frustrating and I haven't even begun swaying lol
