Ok, I've figured it out. Go to the page you posted here in this thread, then scroll down to select the sticky " Girl Attempt Survey System- How to Post Your Sway Info Upon BFP, then embedded in the post, select TTC A Girl Sway Attempts, and you will see the form you need to fill out.
I can't wait to read it!
