    Personalized Swaying Input Questionnaire

    I paid for my personalized plan yesterday and cannot find the questionnaire, I have searched forums and cannot for the life of me figure out where it is so that I can fill it out. Please help me, I need explicit and simple instructions. Thanks, Anangle

    I can't find it either! I found the new white section that says Access to questionnaire but when I click on that there appears to be no questionnaire link, just irrelevant stuff. Please help!
