Where to find customized questionnaire for TTC girl

I just signed up for the customized TTC girl plan for $139.

Where is the questionnaire? When can I expect it and how will I find it?

It sounds like it should show up here:

However, there's only the attempts and free forms available.

I've tried logging out and back in and I've checked my spam folder.
